- The checklist needs to be completed PRIOR to the start date. Completing this checklist on or after the UCS Start Date will be considered late (see example below)
- Answer the questions as a point in time snapshot. Do not wait until all answers are yes to complete the checklist.
- NA should be used for any question that does not apply.
The updated checklist will appear with new schedules starting 3/5/2019. Old checklists are fine and are to be used as normal. You do not need to PURGE or REFRESH schedules to get the new checklist.
